Each year, more than 400,000 patients undergo cardiac surgery in the United States of America and it is estimated that the incidence of post-surgical cardiac arrest is 0.7-8%, and what deserves special mention is its relative good prognosis of Advanced Life Support in Cardiovascular Surgical Units recommended since 2009 by the European Association of Cardiothoracic Surgeons, and the Society for Thoracic Surgery since 2017, and in compliance with the 2020 American Heart Association clinical practice guidelines; And which consists of the timely detection of cardiac arrest, analysis of the rhythm and classifying the rhythm into 3 groups: 1, Shockable rhythms, 2, Asystolia or extreme bradycardia and 3, Electro-mechanical dissociation. Patient with a diagnosis of juxtaductal aortic coarctation with aortic arch hypoplasia, who was admitted to our Pediatric Intensive Care Unit, after surgical correction with interposition of a Woven Dacrón® prosthetic graft in the affected site, which, at 72 hours, presented arrest Cardiac arrest, the Advanced Life Support in Cardiovascular Surgical Units protocol was initiated, which unlike the universal protocol for Advanced Pediatric Life Support recommended by the American Heart Association, All infusions are interrupted, including inotropics, epinephrine doses are not administered, and chest compressions are interrupted to perform the resternotomy, avoiding wasting time in ineffective compressions.
